Wesley Care Finder provides free, personalised support for eligible older guests who may find it difficult to access aged-care services without assistance. A visiting worker can help guests contact My Aged Care, arrange and prepare for assessments, understand forms and service agreements, find suitable providers and connect with additional community support. Soul Hub is a community access point for Wesley Care Finder with a free drop-in service fortnightly on Wednesdays, 10:30am – 12:30pm.

To help guests connect with Wesley Care Finder in a welcoming and safe environment, you can donate to our ‘Convergence of Help’. This community of support relies on funding to provide guests with additional support and mentoring, build connections with local businesses and services, and co-ordinate volunteers.
